How does the PSTN work exactly? (As it relates to VOIP)

First I apologize in advance for these newbie questions, and I did search Google and Wikipedia first.

How does the PSTN work exactly? (As it relates to VOIP)

I understand calls to the PSTN have a per minute charge, who gets the money for that?

If I have a POTS pci card for my server, how do I get service to that? Threw a regular phone company? Wouldn't I have to pay their per minute rates?

So if you calling out, the probably the company providing the line inbetween your VOIP termination providers closest server to the person your calling, and the PSTN line connected to that server and the person you are calling. Is that the right logic?

Keep in mind that the phone

Keep in mind that the phone carried does not know if the calls are coming from a voip phone system or a regular one. You just by the right card for your server to connect the lines you have (digital or analog).

Your rate plan won't change based on the new phone system. You can also use the Internet with trixbox to bypass the phone company all together and send and receive calls to the PSTN via the Internet. To do this you have to subscribe to a Voip phone company such as Bandwidth.com.
