Is it possible to set the font size for Caller-ID name and number for incoming calls?
Also, is it possible to set a background image?

That is not good. Because of the image on the left side (next to the CALLER ID Name field) the place for the caller name is very limited.
Is it possible to disable the caller-picture, so that you get more space for the name and phone number?
Can we send the caller-id alternatively via XML to the phone? Can I change the font formatting via XML?

What you can do: Use an XML TextScreen or FormattedTextScreen to display the Caller ID via XML (using Action URI incming/outgoing). But this XML screen will be an overlay over the existing phone screen, so you lose some of the call control buttons. Also, the Caller ID won't be saved in the callers / redial list doing it that way.
